Prepare for Your Visit
We want your first appointment to go as smoothly as possible. Below you’ll find everything you need to know: what to bring, what to expect, and how to get in touch with us before your visit.
Step 1
Before Your Visit
Complete your new patient intake forms ahead of time (download below) or plan to arrive 15 minutes early. Gather your insurance card, photo ID, medication list, and any referral paperwork or records from your referring doctor.
Step 2
During Your Visit
Expect your first appointment to take 60 to 90 minutes. Dr. Shusko will perform a comprehensive eye examination, which may include specialized imaging and diagnostic testing. You’ll have time to discuss your history, ask questions, and understand next steps.
Step 3
After Your Visit
We’ll send a consultation report to your referring provider. If additional testing or treatment is recommended, our team will help coordinate scheduling. You’ll leave with a clear understanding of your diagnosis and plan.

Uveitis is inflammation inside the eye that can affect the iris, ciliary body, choroid, or retina. It can be caused by autoimmune conditions, infections, or medications, and sometimes the cause is unknown. Without proper treatment, uveitis can lead to vision loss.
Will I need blood work or other testing?
Possibly. Many cases of ocular inflammation are associated with systemic conditions, so blood work and imaging may be necessary to determine the underlying cause and guide treatment. Dr. Shusko will discuss any recommended testing with you during your visit.
How long is the first appointment?
Plan for 60 to 90 minutes. Your first visit includes a comprehensive evaluation with specialized testing, so it takes longer than a routine eye exam.
Do you accept my insurance?
We work with most major medical insurance plans. Because ocular inflammatory conditions are medical conditions, they are covered under your medical insurance rather than a vision plan. Call us and we’ll verify your benefits before your appointment.
Do I need a referral to be seen?
While many of our patients are referred by other doctors, some insurance plans allow self-referral. Contact our office and we can help determine whether you need a referral from your provider.
Can I see Dr. Shusko for a second opinion?
Absolutely. We welcome patients seeking a second opinion on their ocular inflammatory condition. Bring any records and imaging from your previous provider to help us give you the most informed assessment.
